THE CHALLENGE

AlohaCare, a community-led, non-profit health plan, is the third-largest health plan in Hawai’i and is the only health plan dedicated to serving those eligible for Hawai’i’s QUEST Integration (Medicaid) and Medicare program. The organization continually works to improve the health and wellness of its members. However, cumbersome data collection, limited analytics, and gaps in social determinants of health (SDOH) data hindered efforts to understand, quantify, and improve population health.

THE SOLUTION

AlohaCare used the Health Catalyst® data platform and a robust suite of analytics applications, including Pop Insights, to access the data and insights required to improve population health and ensure accurate understanding and reporting of performance. The organization has access to the clinical and claims data required to drive health coordination, including member movement between islands, ensuring care teams can adjust service delivery.

AlohaCare is expanding its whole-person care approach by integrating over 10 SDOH data sources with 20 segmentation views. This integration of data empowers the organization to identify member needs effectively, facilitating proactive outreach and connection to necessary referrals and resources.
