Part of the Ambulatory Suite, the Quality Measures Improvement analytic accelerator provides data on quality measure performance for the providers in your practice, so you can identify where access issues may be causing care gaps and improve access and provider productivity for those patients. This accelerator visualizes data from Health Catalyst’s MeasureAble application (preferable) or from any other measures engine implemented by your organization (if the engine makes measures data accessible).

The Challenge

Health systems that have acquired ambulatory care practices—and independent ambulatory practices alike—face tremendous financial pressures and challenges to financial sustainability. If you’re an ambulatory care leader, you’re looking to improve the efficiency of the services you provide and find cost savings without negatively impacting patient outcomes, the community, or the integrity of the organization. Yet it can be difficult to access data that brings actionable insights to inform decisions to improve patient access, panel management, provider productivity, referral management, or quality scores.

This accelerator visualizes data from Health Catalyst’s MeasureAble application (preferable) or from any other measures engine implemented by your organization (if the engine makes measures data accessible).

A Quality Measures Summary shows overall performance on quality measures, number of care gaps per patient, and current performance for each quality measure. An Analysis view helps you evaluate performance by program, region, or provider and see trends in closing care gaps. A Demographics, SDoH, and Health Equity view helps you evaluate open and closed care gaps in terms of sex at birth, age group, ethnic group, financial class, marital status, and race group.

  • Close quality measure care gaps: Once you have a good understanding of care gaps, you can use Patient Access to improve access for patients who have care gaps, and use and Provider Productivity to improve the productivity of providers in your practice who have care gaps. These visualizations all use the same advanced data model designed for ambulatory care.
