Empowering Care Teams with Real-Time Sepsis Data Decreases Mortality and Costs

Empowering Care Teams with Real-Time Sepsis Data Decreases Mortality and Costs

Summary

Cone Health faced challenges with sepsis care, which was the leading cause of patient mortality and contributed to longer hospital stays. To address this, the organization implemented the Health Catalyst Sepsis Analytics Accelerator, providing care teams with real-time data on patient status and workflow compliance. This enhanced visibility enabled leaders to track performance, identify improvement opportunities, and help teams screen, treat, and intervene sooner, resulting in better sepsis care and improved patient outcomes.

THE CHALLENGE

At Cone Health, sepsis was the leading cause of death among patients and was associated with a longer-than-average length of stay. Leaders and care teams lacked access to real-time data, which prevented them from effectively analyzing performance or identifying improvement opportunities. As a result, leaders struggled to track and report sepsis bundle compliance consistently.

THE SOLUTION

Cone Health implemented the Health Catalyst Sepsis Analytics Accelerator, providing teams with real-time visibility into workflows and patient status. Leaders can now easily track compliance with all sepsis processes, including three- and six-hour bundles. Teams can access the application on demand, utilize it in monthly meetings across campuses, and distribute transparent, systemwide data reviews.

Care teams implemented new processes to screen, treat, and intervene earlier, preventing negative outcomes. Using Healthcare.AI™, leaders and care teams can easily identify the impact of changes on patient outcomes and confirm statistically significant improvements.

THE RESULT

By utilizing the analytics accelerator and real-time analytics, Cone Health substantially improved sepsis care and patient outcomes, including a reduction in mortality and length of stay.

  • 1.7K lives saved, the result of a 38 percent relative decrease in mortality rate over three+ years.
  • $25M cost savings, the result of a 34 percent relative reduction in ED LOS and a 14 percent relative decrease in median inpatient LOS.
  • 44 percent relative increase in sepsis three-hour bundle compliance.
"Health Catalyst helped us move from fragmented reporting to a systemwide view. That transparency allowed us to align leadership, nursing, and frontline teams around measurable goals."
