MACRA Measures & Insights (Product Demo)
MACRA Measures & Insights is an opportunity analysis application which allows organizations to measure their historical performance against MACRA-MIPS measures leading to more meaningful decisions around risk and outcomes. Analyze groups, specialties and providers to determine potential performance on quality measures for any payer. Provides the ability to compare measures across payers to better align the organization strategically.
This application alone is not intended to track performance, only align efforts across all businesses and payers to choose measures and activities based on facts/data. Once measures are determined, then we can use Community Care to do the performance monitoring and any help with extraction of data to enable the client to easily submit. It’s much easier to monitor a set of measures and everyone be focused on those instead of the 271(or more if other payers are added).
Care Team Insights (Product Demo)
Care Team Insights is a dashboard application for leaders in the care management organization to enable daily views of enrollment, utilization, risk, and cost by care team, facility, care program/family. Evaluating the effectiveness of population health and care management programs poses a significant challenge for healthcare organizations. But without the ability to assess and adjust these programs, it is impossible to be accountable for the care of patients. Health Catalyst’s Care Team Insights tool manages and reports on care management programs using metrics and measures appropriate to value-based contracting. Organizations can determine true Return on Engagement—for example, that $17 in savings on cost utilization is achieved for every $5 spent on complex care program.
Care Companion (Product Demo)
Care Companion is a mobile phone application for patients which supports the engagement of patients with the Care Management team of nurses, pharmacists, social workers, and others in the process of improving individual patient outcomes. Most patients have experienced the frustration of not knowing their care plan nor how to become engaged to ensure the plan’s success. Health Catalyst’s patient engagement tools solve this problem by leveraging smart phones and mobile connectivity. The tools enable secure messaging, assessments, care planning, and the associated activities and education that engage patients in collaboration with their care team. Family, friends, or caregivers can be invited to share in these plans and communications to ensure patients receive the best care.
Patient Intake (Product Demo)
Patient Intake is built to streamline the process of patient intake and care team assignment. It delivers an efficient way of consolidating and managing multiple lists, collaborating with the physician and reaching out to these patients so the real work of delivering care can be done. Users can add, update or remove patients from the list before routing the patient record to the next person in the workflow. This application is required when deploying Patient Stratification as a patient list source for Care Coordination.
Care Coordination (Product Demo)
Care Coordination is a mobile, tablet-based application, used at the point of care by care coordinators and team members to organize patient interventions including shared decision making for patient goals and activities, patient and team communications, as well as alerts and notifications for new admissions or decreasing patient engagement activity. It is important that the entire care team, along with the patient, and the patient’s family and friends, can communicate through the care management solution to develop relationships that help or encourage patient engagement. Health Catalyst’s mobile-first approach enables the care team to go where the patients are: their homes, physician offices, post-acute and long-term care settings. The solution supports all members of the care team including social workers, community resources, care navigators, etc. across multiple EMR systems.
Patient Stratification (Product Demo)
Patient Stratification integrates current and cost trends, chronic conditions, and social determinants risk models and disparate sources to identify the individuals most likely to benefit from proactive care management programs. Users can build and analyze different stratification algorithms based on proven risk models and patient utilization to rank and ultimately determine the most important candidates for intervention through complex care management, chronic condition management, readmission prevention or other programs. Current care management solutions deliver static lists of patients who meet certain population health criteria, without actionable information on how to treat them. Health Catalyst’s Patient Impact Predictor™ enables identification of people who may fall anywhere along three levels of care management but who are most likely to benefit from specific interventions that have worked effectively in the past for similar patient types.
Risk Model Analyzer (Product Demo)
Risk Model Analyzer is aimed at clinical directors and operational directors. The application helps users visualize underlying risk models that calculate risk for a patient at a particular point in time. The risk models currently contained within the application include the Health and Human Services derivation of the HCC model (HHS-HCC), Charlson/Deyo, and APRDRG severity of illness. Risk models can be evaluated from a population or patient standpoint as well as comparatively to determine how high risk patients in one model correlate with high risk patients in another model.
Department Explorer: Surgical Services (Product Demo)
Department Explorer: Surgical Services is an analytics tool that provides managers multiple views of data regarding operational efficiency, utilization, and financial performance specific to the operating room. This information helps users to better analyze specific OR data such as patient prep, anesthesia prep, room turnaround, room and staff scheduling, and procedure data. Separating OR data from other hospital functions provides for specific complex processes to be analyzed along unique OR process paths. This leads to a better understanding, analysis, and improvement opportunities in the areas of cost savings, utilization, and throughput.
Sepsis (Product Demo)
Sepsis Improvement application is aimed at clinical and medical directors, operational directors, clinical program guidance teams in emergency, intensive care, and inpatient units. The application focuses on providing data to help improve early recognition of sepsis, early intervention for severe sepsis, and early therapy for septic shock to reduce mortality, morbidity, and cost.
Patient Experience Explorer (Product Demo)
Patient Experience Explorer delivers multiple, robust views into the data your patients are providing about their healthcare experience. Whether it is summary performance scores for executive leaders, survey results with goals and percentiles for unit managers or survey details for individual providers, this application delivers the information your organization will need in order to understand and improve on the safety, physical comfort, education and other aspects of care being provided to your patients.

Leakage and Referrals is an analytic tool to support more effective network management. The tool is designed to help health care organizations evaluate referral patterns and out-of-network service utilization to identify opportunities for more effectively managing their networks.
Bundle Payments (Product Demo)
Bundled Payments analytics tool evaluates cost and variation associated with care delivery for patients, and is intended to prioritize areas of focus and provide a baseline for exploratory analysis. The application is modeled on the Centers for Medicare and Medicaid Services (CMS) Bundled Payment for Care Improvement (BPCI) Initiative.
ACO MSSP Measures (Product Demo)
ACO MSSP Measures tool supports accountable care organizations (ACO’s) participating in the Medicare Shared Savings Program (MSSP). This tool supports monitoring and managing actual performance against CMS required measures for ACO’s throughout the year. The ability to measure performance and drill into each measure anytime, not only after a required reporting period, provides the ability to identify inefficiencies and opportunities for improvement in a timelier manner.
ACO Explorer (Product Demo)
ACO Explorer is an executive-level dashboard for monitoring ACO health. Review trends and performance against targets on key metrics for per-member-per-month performance, leakage, and utilization. The performance dashboard will include ACO measure performance if the organization has deployed ACO Measures as well.
Practice Management: Patient Access
Practice Management: Patient Access helps provide big-picture insight about the patient-access element of a medical practice. What are the monthly and yearly trends for completed appointments? What about no shows or the numbers of patients who left without being seen? What about cancellations and a break-down of the reasons for all cancellations?
